Article 884.7
La demande pour constater la prise d’effet d’un mandat donné par une personne en prévision de son inaptitude, la déclaration de cessation des effets ou la révocation d’un tel mandat, peut également être présentée à un notaire.
Le notaire signifie la demande au mandant et, le cas échéant, la notifie au mandataire ainsi qu’au mandataire substitut désigné par le mandant, au curateur public et à l’une des personnes visées dans l’article 15 du Code civil.

Article 884.7
An application for the recording of the coming into effect of a mandate given in anticipation of the mandator’s incapacity or of the declaration of the cessation of the effects or the revocation of such a mandate may also be presented to a notary.
The application is served by the notary on the mandator and, where applicable, notified to the mandatary and to the substitute mandatary designated by the mandator, the Public Curator and one of the persons mentioned in article 15 of the Civil Code.
